Why Your Cholesterol Test
Might Be Lying to You —
And What to Do About It

The standard lipid panel most Australians receive from their GP tells only part of the story. Here's what you actually need to know to protect your heart — naturally.

Domenic Pisanelli · Naturopath & Functional Medicine Practitioner · 8 min read

Cardiovascular disease claims one Australian life every four minutes. Despite this, the advice most of us receive about cholesterol is oversimplified, often outdated, and — frankly — incomplete. If you've been told your cholesterol is "fine" or "a bit high," there's a very good chance the full picture hasn't been painted for you.

Let's change that. In this article, I'll walk you through what cholesterol and lipoproteins actually are, why the "good vs bad" framework is misleading, what's really driving cardiovascular risk, and — most importantly — what you can do naturally to optimise your heart health using our ROOT Method™ framework.

The "Good vs Bad" Cholesterol Myth

You've heard it a thousand times: HDL is "good cholesterol," LDL is "bad cholesterol." This framework is simple, memorable — and fundamentally misleading.

Cholesterol itself is not good or bad. It's a molecule your body cannot live without. It builds every cell membrane in your body, synthesises your sex hormones (oestrogen, testosterone, progesterone), produces bile acids for fat digestion, enables vitamin D production in your skin, and provides the structural basis of the brain's myelin sheaths. Without adequate cholesterol, your hormonal health, brain function, and immunity would collapse.

The more accurate framework asks: how many atherogenic (plaque-forming) lipoprotein particles are circulating in your bloodstream, and what is the inflammatory environment those particles are operating in? That question cannot be answered by a standard cholesterol reading alone.

🔬 The Critical Gap in Standard Testing

A person can have a "normal" LDL cholesterol of 2.8 mmol/L and still carry a dangerously elevated LDL particle number — because small, dense LDL particles carry less cholesterol individually but are far more atherogenic. Standard Medicare-funded panels measure cholesterol content, not particle number. This discordance affects a significant proportion of Australians with insulin resistance or metabolic syndrome — and it goes undetected every day.

What Actually Drives Heart Disease

Atherosclerotic cardiovascular disease is not simply a disease of cholesterol accumulation. It is a chronic inflammatory disease of the arterial wall. Here's what actually happens:

ApoB-containing lipoprotein particles — primarily LDL, but also VLDL remnants and Lp(a) — penetrate the artery's inner lining and become retained in the arterial wall. Once inside, they undergo oxidative modification. Immune cells (macrophages) engulf the oxidised particles, transforming into foam cells that accumulate into fatty streaks — the earliest lesion of atherosclerosis. Over decades, plaques grow, fibrous caps form, and eventually, rupture triggers the acute heart attack or stroke.

The key insight: cardiovascular risk is about the number of atherogenic particles attempting to cross the arterial wall over your lifetime — not simply the amount of cholesterol floating in your blood. This is why markers like ApoB, LDL particle number (LDL-P), and Lp(a) matter so much more than total cholesterol or LDL-C in isolation.

📋 What You Should Actually Test

Beyond a standard lipid panel, ask your practitioner about: ApoB (counts all atherogenic particles), Lp(a) (a genetically determined, often-missed risk factor), hsCRP (systemic inflammation marker), fasting insulin and HOMA-IR (to assess insulin resistance — a primary driver of atherogenic dyslipidaemia), and non-HDL cholesterol. For those in the right age and risk category, a Coronary Artery Calcium (CAC) score is among the most powerful tools available.

10 Natural Strategies That Actually Move the Needle

The following evidence-based strategies form the backbone of the OPTIMIZE, ORGANIZE & REBUILD, and TRANSFORM phases of our ROOT Method™ programme. They work best as a comprehensive system — not cherry-picked in isolation.

  1. 1
Address insulin resistance first. It's the primary driver of atherogenic dyslipidaemia — high LDL particle number, elevated triglycerides, and impaired HDL function. Reducing refined carbohydrates, improving sleep, and exercising regularly have a profound ripple effect across your entire lipid profile.
  2. 2
Adopt a Mediterranean-style dietary pattern. Rich in extra virgin olive oil, vegetables, legumes, oily fish, and nuts — this pattern has the strongest cardiovascular outcome data of any diet and aligns beautifully with what's available across Australia.
  3. 3
Prioritise soluble fibre. Oats, barley, psyllium husk, legumes, and flaxseed bind bile acids in the gut and force the liver to pull cholesterol from circulation — a mechanism similar to pharmaceutical bile acid sequestrants, without the side effects. Target 10–25g of soluble fibre daily.
  4. 4
Make quality extra virgin olive oil your primary fat. Its polyphenols (particularly oleocanthal and oleuropein) reduce LDL oxidation, improve HDL function, and lower ApoB levels. Choose cold-pressed, certified Australian EVOO and aim for 2–4 tablespoons daily.
  5. 5
Combine aerobic exercise with resistance training. Aerobic exercise raises HDL and lowers triglycerides. Resistance training reduces small dense LDL and improves insulin sensitivity. Together, they're superior to either alone — and three 30-minute sessions per week can make a meaningful difference.
  6. 6
Treat stress as a clinical priority. Chronic stress elevates cortisol, promotes hepatic cholesterol synthesis, and drives visceral fat accumulation — all of which worsen your lipid profile. Breathwork, mindfulness, and adequate time in nature are not "soft" interventions — they have measurable cardiometabolic effects.
  7. 7
Protect your sleep. Under 6 hours of sleep per night is independently associated with dyslipidaemia, insulin resistance, and elevated inflammatory markers. Sleep apnoea — significantly underdiagnosed in Australia — is a major unaddressed driver of poor lipid health for many people.
  8. 8
Reduce alcohol significantly. For those with elevated triglycerides, alcohol is often the primary driver. The 2023 World Heart Federation position is unambiguous: there is no safe level of alcohol for cardiovascular health.
  9. 9
Consider targeted supplementation. Bergamot polyphenolic fraction, omega-3 fatty acids (EPA/DHA), berberine, and plant sterols all have strong clinical evidence for improving lipid parameters. The right combination depends on your specific lipid pattern — which is why testing and personalisation matter.
  10. 10
Address nutritional deficiencies. Magnesium, vitamin D, B vitamins, zinc, and selenium all play direct roles in lipid metabolism, insulin sensitivity, and cardiovascular protection — and deficiencies are common in Australians eating a modern diet.

About Cholesterol Medications

Pharmaceutical therapy — statins, PCSK9 inhibitors, ezetimibe, and others — has a legitimate and sometimes life-saving role in managing lipid health, particularly for those with familial hypercholesterolaemia or established cardiovascular disease. We never recommend stopping prescribed medications without medical supervision, and we work collaboratively with our clients' GPs and specialists.

What we do advocate is that medication decisions should be individualised, informed, and accompanied by genuine lifestyle and nutritional support — not a replacement for it. For many Australians at low to intermediate cardiovascular risk, comprehensive natural intervention produces meaningful, lasting results without the need for pharmaceutical therapy.
