Your cholesterol is elevated. Your GP has offered statins. You're not necessarily against medication — but you want to understand why it's happened before committing to a drug you'll likely need indefinitely.

That's a reasonable position. And it's exactly where naturopathic functional medicine has something concrete to offer.

I'm Domenic Pisanelli, principal naturopath at Vital Health and Natural Medicine in Kealba, Melbourne. I've been working with patients on elevated cholesterol, metabolic dysfunction, and cardiovascular risk for over 25 years. This page explains how I approach elevated cholesterol differently — what I test, why I test it, and what that means for treatment.

Cholesterol Is a Symptom. The Question Is — Of What?

Conventional medicine often focuses on reducing cardiovascular risk by lowering cholesterol levels. Functional medicine additionally asks what may be driving the elevation — and whether that underlying cause is being addressed.

Cholesterol is produced by the liver in response to metabolic signals. When those signals are dysregulated, the liver overproduces. Statins interrupt that production. They work. But they don't change the underlying conditions driving it.

In clinical practice, I consistently see elevated cholesterol linked to one or more of the following:

  • Insulin Resistance Elevated fasting insulin signals the liver to increase cholesterol output. This is the most common driver I see, particularly in patients with central adiposity, energy crashes, and carbohydrate cravings.
  • Chronic Systemic Inflammation Roughly 70% of cholesterol is produced hepatically in response to inflammatory burden. Gut dysbiosis, food reactivity, and elevated hs-CRP are common contributors.
  • Subclinical Hypothyroidism Even a TSH in the "normal" range can mask impaired T3 conversion, which slows cholesterol clearance from the bloodstream. This is frequently missed on standard panels.
  • HPA Axis Dysfunction Chronic cortisol elevation drives insulin resistance and visceral fat accumulation, both of which compound lipid dysregulation.
  • Nutrient Deficiencies Magnesium, vitamin D, and B vitamins are all involved in lipid metabolism. Deficiency is frequently overlooked in standard workups.

None of these appear on a standard cholesterol panel. All of them are testable.

What Functional Testing Actually Reveals

A standard lipid panel tells you your total cholesterol, LDL, HDL, and triglycerides. That's a starting point — not a diagnosis.

In most cases, I order additional functional testing to identify what's actually driving the picture:

  • Fasting Insulin and Glucose Assesses insulin resistance before it shows up as type 2 diabetes. A fasting glucose of 5.5 can coexist with a fasting insulin of 18, indicating significant metabolic dysfunction that a glucose-only test misses.
  • Full Thyroid Panel Not just TSH — free T3, free T4, reverse T3, and thyroid antibodies. A patient with TSH of 2.8 can still have impaired conversion and all the metabolic consequences of low active thyroid hormone. This connects directly to hormonal health.
  • Inflammatory Markers — hs-CRP and Homocysteine These tell me whether inflammation is a significant driver of the lipid picture and quantify cardiovascular risk more accurately than LDL alone.
  • Organic Acids Test (OAT) A first-morning urine test assessing cellular energy production, mitochondrial function, neurotransmitter metabolism, and markers of bacterial and yeast overgrowth. One of the most information-dense tests I use.
  • GI Microbiome Map Stool analysis assessing gut microbiome diversity, inflammatory markers, digestive enzyme output, and pathogenic load. Gut inflammation drives systemic inflammation, which drives cholesterol.
  • AdrenoCortex Salivary Panel Diurnal cortisol curve across four time points. This tells me whether HPA axis dysfunction is contributing to insulin resistance and visceral fat accumulation.

Testing is not mandatory for every patient. But without it, you're treating a number rather than a person.

The ROOT Method™ — My Clinical Framework

The ROOT Method™ is the four-phase framework I use to assess and treat complex presentations like metabolic and cardiovascular dysfunction. Every protocol is built from your specific test results — not a generic template.

R
Review

Full health history, symptom mapping, and functional test results interpreted in context — not in isolation.

O
Order

Targeted functional testing based on your clinical picture, not a blanket panel.

O
Observe

Pattern recognition across systems: gut, thyroid, adrenal, metabolic, immune.

T
Treat

Phased protocol addressing root causes first, with nutritional, supplemental, and lifestyle interventions matched to your biochemistry.

What a Natural Cholesterol Protocol Looks Like

Once I understand what's driving elevated cholesterol, treatment targets those specific mechanisms. Below is a representative overview — individual protocols vary based on test results.

Dietary Intervention

Focuses on removing inflammatory inputs — refined carbohydrates, industrial seed oils, processed foods — and introducing foods that support lipid metabolism: fatty fish, extra-virgin olive oil, avocado, ground flaxseed, soluble fibre, and fermented foods for gut health. For patients with significant insulin resistance, a lower-carbohydrate approach is often the highest-leverage dietary change in the first 8 weeks.

Targeted Supplementation

Prescribed at therapeutic doses based on test findings. All practitioner-grade formulations. Common evidence-based nutrients in this context:

  • Omega-3 fatty acids (EPA/DHA) — anti-inflammatory, directly lowers triglycerides
  • Berberine — comparable to metformin for insulin sensitisation; also supports lipid metabolism directly
  • Plant sterols — reduces intestinal cholesterol absorption
  • Bergamot extract — well-studied for LDL reduction and HDL improvement
  • Selenium and zinc — critical for thyroid hormone conversion where indicated
  • Magnesium glycinate — involved in hundreds of enzymatic pathways including glucose metabolism
  • Vitamin D3 — frequently deficient; impacts immune regulation and metabolic function
  • CoQ10 — particularly relevant if statins are also being used, as statins deplete CoQ10

Lifestyle Prescription

Includes sleep optimisation (disrupted sleep drives cortisol dysregulation and insulin resistance), movement protocols matched to cortisol status — excessive HIIT is counterproductive in HPA dysregulation — and targeted stress load reduction. All adjusted across follow-up appointments as markers respond.

Is This Instead of Statins?

Not necessarily. This depends on your individual risk profile, which your GP is best placed to assess.

For patients with familial hypercholesterolemia, previous cardiovascular events, or very high LDL requiring immediate intervention, statins are appropriate. I work alongside your GP — not against them.

For patients with lifestyle-driven metabolic dysfunction — insulin resistance, gut inflammation, thyroid underfunction, cortisol dysregulation — addressing root causes can produce significant and sustained improvements in lipid markers, often reducing or in some cases removing the need for medication over time. That decision is always made collaboratively with your prescribing doctor.

What I can tell you is that lowering the number without addressing the cause means the underlying dysfunction continues. For many patients, that manifests downstream as blood pressure issues, blood sugar problems, or fatigue — all of which could have been caught earlier.

Who I Work With

This approach works best for patients who:

  • Have elevated cholesterol and want to understand the root cause before starting long-term medication
  • Have been on statins but continue to have other symptoms — fatigue, weight resistance, gut issues, poor sleep — and suspect there's more going on
  • Have borderline or mildly elevated cholesterol and want to address it proactively through a targeted, tested approach
  • Have tried "eating better" without meaningful results and want to understand why

I see patients in-clinic in Kealba (Melbourne's north-west) and via telehealth nationally. If you're looking for a naturopath near you in Melbourne, telehealth means location is rarely a barrier.

Frequently Asked Questions

For many patients with lifestyle-driven elevated cholesterol, yes — significant improvements are achievable through dietary change, targeted supplementation, and addressing root causes like insulin resistance or gut inflammation. For patients with genetic conditions or very high cardiovascular risk, medication may still be necessary. The right answer depends on your specific results and risk profile.
Meaningful changes in lipid markers typically appear within 4–8 weeks when protocols are well-targeted. Full metabolic restoration — including gut health, insulin sensitivity, and thyroid function — takes longer, often 3–6 months. Most patients notice improvements in energy, sleep, and digestion before the labs shift.
No. I do not advise patients to stop prescribed medication. If your results improve significantly over time and your GP agrees, that conversation can happen — but it's always their call. The ROOT Method works alongside conventional treatment, not instead of it.
Depends on your clinical picture. Commonly: fasting insulin, full thyroid panel, hs-CRP, homocysteine, Organic Acids Test, GI Microbiome Map, and AdrenoCortex Salivary Panel. I'll recommend what's clinically indicated at your initial consultation.
Yes. I can communicate with your GP directly and work within an integrated care model. I don't replace your medical team — I work alongside them.
90 minutes. Full health history, symptom mapping, review of any existing pathology results, and a plan for functional testing where indicated. Initial consultation fee is $197.
